Background
The Centre for Chronic Disease Management is a collaboration between Monash University’s School of Nursing and Midwifery, Graduate Medical School, Department of Rural and Indigenous Health, partnering with Latrobe Regional Hospital and Latrobe Community Health Service. This mix of collaborators brings a wealth of research experience across chronic disease management and ensures that evidence can be implemented at the point of care.

The aim of the Centre is to promote the development and implementation of evidence-based health care in chronic disease. The Centre has a special interest in the impact of chronic disease in regional and rural communities and health services.
