Registration

Please read through the following information before completing the Registration Form. All those wishing to attend the conference must complete the form and return it, with full payment, as indicated on the form.

On receipt of payment, a confirmation letter and tax invoice will be sent to you. Please keep this letter as your official receipt.

Early Bird registration fees

To qualify for early bird registration fees, registrations must be made by close of business on Tuesday 30 June 2009. If payment is not received within two weeks of this date, the early bird discount will not be available and the higher rate will apply.

Full registration includes:

Entry to all sessions and displays
Morning/afternoon tea and lunch
Conference satchel and final program
One Conference Gala dinner ticket

Student Registration

Available to a researcher, students or other delegate earning less that A$45,000 per annum. Verification from the Head of Department must be forwarded with the registration form to enable the application to be processed. Students only need to forward a photocopy of their current student card with their registration form to receive the discount.

Day Registration includes:

Entry to all sessions and displays on the day of attendance
Morning/afternoon teas and lunches on the day of attendance
Conference satchel and final program
Wednesday Registration includes a ticket to the Gala Dinner

Extra tickets to Gala Dinner are available for $150.00 each (Limited seats available) Please contact Chris Cafcakis via chris.cafcakis@adelaide.edu.au to arrange extra tickets.

PAYMENT OF FEES:

All prices are quoted in Australian Dollars and inclusive of GST. Payment in Australian dollars must accompany all registration forms and can be made by cheque, international bank draft drawn by and Australian bank or credit card.

If payment is being processed through a large organisation and may be delayed, please forward a separate copy for preliminary registration and ask your accounts section to ensure that the original form is forwarded with payment. However, registrations will only be acknowledged once payment has been received. We suggest you make a copy of this form for your own records.

Please note that debits to your credit card, excluding accommodation, will appear as ‘Royal Adelaide Hospital’ on your credit card statement.

TAXES

Australia has a federal goods and services tax (GST) of 10% which is included in the registration fee. Overseas delegates are not exempt from paying this tax.

CANCELLATION POLICY

Cancellations must be made in writing to the secretariat. Cancellations received before 15 October 2009, will be refunded less an administration fee of $85. Cancellations made after this date will not be eligible for a refund, although registrations are transferable. Refunds will only be issued in exceptional circumstance and at the discretion of the organising committee. No refunds will be given for non-attendance.

ACKNOWLEDGEMENT

All registrations will be acknowledged in writing and will include a summary of registration requirements as indicated on your registration form. A receipt of payment and tax invoice will also be included. Please check your confirmation letter carefully and notify the secretariat immediately of any changes required. If you have not received an acknowledgement three weeks after sending your details, we suggest that you contact the secretariat to ensure that your form has been received
